Care MAP Tool: A Business Case Modeling Tool for Complex Care Management
The Care Management Assessment and Planning (Care MAP) Tool is a business case modeling tool developed with the goal of guiding clinics through the considerations involved in designing a sustainable complex care management (CCM) program. The tool provides a structure for assessing multiple parts of CCM programs, including patient and visit volumes, staffing models, expenses, revenues, impact, and value. It focuses on complex care management supports for high risk patients (patients in the top 5-20% of complex needs), for which current care management revenues are limited.
Care MAP was created by the Institute for High Quality Care (IHQC) with the generous support of Cedars-Sinai. IHQC also thanks Chapman Consulting, CoMagine Health, and several Los Angeles area community health center focus-group participants for their input, expertise and guidance on the Care MAP tool.
While the tool could be leveraged by a variety of healthcare roles, it was intentionally developed to help program managers, quality teams, and/or clinic leaders consider some of the factors and variables that would contribute to the overall business case for complex care management.
Care MAP Components
The Care MAP Tool is an Excel workbook file that includes six different worksheets to guide the user through defining key business case components, inlcuding:
- Understanding Your Patient Population
- Projecting Encounter Volumes & Staffing Needs
- Program Expenses
- Program Revenues
- Program Impact & Value
- Executive Summary & Value Proposition Reflections
In addition to the downloadable Care MAP tool, IHQC has created two video modules to help acclimate and guide users through template. A resource library is also available with curated tools, references, and case scenarios to allow users to dive deeper into each business case component.
